Position Summary and Responsibilities: The
Community Outreach Worker connects individuals and families to health education, resources, and supportive services that improve wellness and reduce disparities. This role includes conducting home visits, organizing health events, and providing culturally responsive counseling on topics like prescription literacy, nutrition, and chronic disease management. The Outreach Worker also links community members to local resources, advocates for health equity, and gathers data to strengthen programs and outcomes
Key Responsibilities Outreach and Engagement: Visit the homes of community members to complete patient intake paperwork. Organize and facilitate community health events, workshops, and presentations. Collaborate with local organizations and agencies to enhance community health initiatives.
Health Education: Provide health education on various topics such as prescription literacy, nutrition, preventive care, chronic disease management, and mental health. Offer one-on-one counseling sessions to address individual health concerns and promote healthy lifestyle choices. Develop culturally sensitive educational materials tailored to the needs of the community.
Resource Referral: Assist community members in accessing healthcare services, insurance coverage, and social support programs. Connect individuals to local resources such as food banks, housing assistance, and transportation services. Serve as a liaison between community members and healthcare providers to ensure continuity of care.
Advocacy and Empowerment: Advocate for policies and programs that address the social determinants of health and reduce health disparities. Empower community members to become active participants in their healthcare decisions and self-care practices. Promote health equity by addressing barriers to healthcare access and addressing systemic issues impacting the community.
Data Collection and Reporting: Maintain accurate records of interactions with community
members and services provided. Compile data on community health trends, needs, and outcomes. Generate reports to assess the effectiveness of community health interventions and inform programmatic improvements.
Administrative and Team Support: Complete administrative tasks necessary to advance program operations and organizational goals. Carry out other duties as assigned by the Outreach Manager and CEO.
Required Qualifications: High school diploma or equivalent
Valid Ohio Drivers License and comfort traveling throughout the region
Previous experience in community outreach, healthcare, or social services
- Demonstrated ability to work with culturally diverse and medically vulnerable populations
Ability to work independently and as part of a team
Proficiency in Microsoft Office for data entry and reporting
Willingness to work flexible hours, including evenings and weekends
Assist the Executive Director as necessary
Support the Pharmacist and Front Office team as necessary
Preferred Qualifications Certified Community Health Worker
Certified Medical Assistant
Certified Pharmacy Tech
- Spanish language proficiency or multilingual abilities
Schedule & Working Conditions - Typical schedule is Monday through Friday, 8:00 AM5:00 PM, with flexibility for evening and or weekend outreach events
- Includes frequent travel across Northeast Ohio and working from both mobile units and partner sites
- Must be able to stand for extended periods, lift up to 35 pounds, and work in variable environments such as shelters or outdoor community events
- Salary commensurate with education and experience
