Job Title: Hospice Certified Nursing Assistant (CNA)

Location: Georgia (Service areas covered: Cherokee, Cobb, Douglas, Carroll, Coweta, Fayette, Clayton, Henry, Newton, Rockdale, Dekalb, Gwinnett, Fulton)

Job Type: Full-Time & Part-Time / PRN available

Job Summary:
Our client is seeking a compassionate and dedicated Hospice Certified Nursing Assistant (CNA) to join their team in Georgia. The Hospice CNA provides essential support to patients facing terminal illnesses, offering personal care and emotional support to ensure comfort, dignity, and the highest possible quality of life. This role works closely with the interdisciplinary care team, patients, and families in home settings, hospice facilities, or long-term care environments. Notice: This position requires heavy travel / driving in the service area during the day. Do NOT apply if you do not enjoy driving.

Key Responsibilities:

• Provide personal care services, including bathing, dressing, grooming, oral hygiene, toileting, and other activities of daily living (ADLs) per the patient's individualized care plan.
• Assist with mobility, transfers, and positioning to ensure patient comfort and prevent complications such as bedsores.
• Monitor and report changes in the patient's condition to the RN Case Manager or supervising nurse.
• Offer emotional support and companionship to patients and their families.
• Maintain a clean, safe, and supportive environment for the patient.
• Provide basic care, such as checking vital signs and assisting with nutrition and hydration.
• Ensure patient dignity and respect are upheld at all times.
• Document care provided accurately and in a timely manner, including bedside documentation required in the Homecare Homebase (HCHB) electronic health record (EHR) system.
• Adhere to hospice care regulations and guidelines while following agency policies and procedures.

Qualifications:

• High School Diploma or GED.
• Current and active Certified Nursing Assistant (CNA) certification in Georgia.
• CPR and First Aid certification preferred.
• Minimum of 2 years of hospice (field) experience required.
• Previous experience in palliative care or home health is a plus.
• Strong interpersonal and communication skills.
• Ability to handle the physical and emotional demands of hospice care.
• Reliable transportation, valid driver's license, and auto insurance for travel to patient locations.
• 100% travel to patient homes and facilities is required. Must have reliable transportation and be able to travel within the full hospice service area.
